Do "narcissists" deliberately hurt those closest to them?

People with Narcissistic Personality Disorder and narcissistic traits often hurt those closest to them and they are often characterised as evil, demonic, manipulative and predatory.
But is this a fair reflection? I don’t think so. Most narcissistic behaviour is defensive and designed to protect the narcissistic person from shame, rejection and humiliation.
Whilst hurtful behaviours may not be as intentional or planned as they might appear, that doesn't necessarily lessen the pain and the hurt that has been caused. One of the most tragic aspects of narcissism, is that those who suffer with pathological narcissism, often create the same hurt and pain in those closest to them as they feel themselves: alone, invisible, not good enough.
